Job summary

First Choice Community Healthcare is seeking a Referral Specialist I to track provider-directed referrals and document outcomes. This role involves coordinating with patients, providers, and staff to ensure referrals are processed efficiently.

The position supports scheduling, insurance verification, and related tasks to ensure FCCH patients receive timely referrals. Spanish bilingual skills are highly preferred, and prior use of EHR/practice management systems is advantageous.

Qualifications

• High school diploma or GED with 1+ year of medical office or related healthcare experience.
• Bilingual Spanish-speaking skills highly preferred; experience with multiple providers and EHR/practice management systems preferred.
• CPR certification required within 6 months of hire; medical assisting skills/certification/experience highly preferred.

Responsibilities

• Maintains ongoing tracking of provider-directed referrals and documents status.
• Ensures complete and accurate patient registration data.
• Communicates with patients, providers and staff about referrals.
• May contact insurance for prior approvals and provide needed patient information.
• May schedule patient referral appointments and prepare necessary paperwork.
• Attends meetings as required and performs miscellaneous duties as assigned.
