Job summary

Kaniksu Community Health in Idaho seeks a Behavioral Health Case Manager to support patients by linking them to health and social services through our PCMH team. This role emphasizes collaboration with behavioral health providers and patient-centered care within a community health setting.

You will work under the Behavioral Health Director, performing triage, care coordination, and crisis support while keeping client files and notes updated and confidential.

Qualifications

  • Bachelor’s degree in health or human services and experience in the field of mental health.
  • Working knowledge of administrative procedures in federally funded community health centers (preferred).
  • Computer skills to maintain client charts and communications within the organization.
  • Must pass Idaho Department of Health and Welfare background check (enhanced).
  • Knowledge of Medicaid/Optum/Managed Care regulations (preferred).
  • Participate in reciprocal feedback from treatment team members.
  • Adhere to the NASW code of ethics.
  • Knowledge of community resources and networking skills.

Responsibilities

  • Provide triage support for individuals identified by medical or mental health providers as needing supportive health/social services.
  • Work as a treatment team member.
  • Provide crisis response and support when needed.
  • Maintain electronic client files and mental health screenings/encounter notes.
  • Coordinate identified care needs with supportive services.
  • Ensure confidentiality per policy and protect client rights.
  • Communicate with treatment team members.
  • Make CPS referrals when necessary.
  • Follow Professional Code of Ethics.
  • Attend staff/Behavioral Health meetings and other necessary meetings.
  • Maintain regular and predictable attendance.
  • Carry out other related duties as assigned.

At Kaniksu Community Health, the Behavioral Health Case Manager is not just responsible for providing excellent customer service. They are committed to working closely with Behavioral Health care providers to help identify what type(s) of additional health/social services an individual is in need of and connects the patient to these supports. YOU ARE an essential team member charged with assisting patients in managing their care at Kaniksu Community Health.

Under the direction of the KCH Director of Behavioral Health and Behavioral Health Providers and as a member of the Kaniksu Community Health Patient Centered Medical Home (PCMH) Team, a typical day of the Behavioral Health Case Manager might include;

  • Provides appropriate triage support for individuals identified by medical or mental health providers as being in need of supportive health/social services.
  • Works as a treatment team member.
  • Provides crisis response and support when needed.
  • Maintains electronic client files, including mental health screenings and encounter notes.
  • Coordination of identified care needs with appropriate supportive services.
  • Compliance with confidentiality policy as it applies to protection of client's rights.
  • Communicates with the members of the treatment team.
  • Makes CPS (Child Protection Services) referrals when necessary.
  • Strict adherence to Professional Code of Ethics for appropriate discipline. (if not aware of this Code, must obtain a copy and become familiar with it)
  • Provides all services in a professional and respectful manner at all times.
  • Understanding and adherence to all Kaniksu Community Health Policies and Procedures.
  • Attends all staff, Behavioral Health meetings and other meetings as necessary.
  • Regular and predictable attendance is an essential function of this position.
  • Carries out other related duties as assigned.
Experience Needed to Land this Gig:
  • Bachelor’s degree in health or human services and experience in the field of mental health
  • Working knowledge of administrative procedure of a federally funded community health center, preferred.
  • Computer skills necessary to maintain client charts and communication within the organization.
  • Must be able to pass Idaho Department of Health and Welfare Criminal History Unit enhanced background check.
  • Knowledge of Medicaid/Optum/Managed Care regulations preferred.
  • Participate in reciprocal feedback from treatment team members.
  • Adhere to the NASW code of ethics.
  • Knowledge of community resources and networking skills.
Work Schedule:
  • Full Time
  • 32-40 hours per week
