Job summary

Community-Health-of-Central-Washington (CHCW) in Yakima, WA is seeking a Referral Coordinator to coordinate patient referrals and support healthcare providers. You will help ensure referrals are processed accurately, efficiently, and in a timely manner, guiding patients through next steps in their care journey.

The position is full-time, Monday–Friday, with a bilingual differential for Spanish/English and comprehensive benefits.

Qualifications

• HS Diploma/GED is required
• 2 years related experience is required
• Driver’s license and auto insurance is required
• Bilingual Spanish/English is preferred and a plus!
• Immunizations are required prior to start date (COVID, Hep B (3), Tdap, MMR (2), Varicella (2), TB/PPD, FLU)
• Pre-employment drug screen is required

Are you organized, detail-oriented, and passionate about helping patients access the care they need? CHCW is looking for a dedicated Referral Coordinator to join our team! In this role, you’ll play an important part in coordinating patient referrals, supporting our healthcare providers, and ensuring patients can navigate the next steps in their healthcare journey with confidence. Working closely with providers, clinic teams, insurance companies, and specialists, you’ll help ensure referrals are processed accurately, efficiently, and in a timely manner. Qualifications
