Under the direction of the assigned manager or coordinator, receives patient referrals from providers or nursing personnel, ascertains their needs and makes appropriate referral. Documents the referral appropriately in referral log and communicates with provider that referral has been made. Has the ability to perform duties consistent with Standards of Practice, goals and philosophy of Ozarks Community Hospital and the Employee Code of Conduct.
