Job summary

BAYADA Home Health Care is seeking an Associate/Referral Coordinator to join our Canton, MA Medicare certified Home Health Visits Office. The role emphasizes superior client service, scheduling support, and building relationships with clients, referral sources, and payors.

Ideal candidates hold an Associate degree (Bachelor preferred) and have familiarity with Medicare, OASIS, and electronic medical records, plus excellent communication.

Qualifications

• Bachelor degree preferred; Associates at minimum.
• Medicare and OASIS knowledge a plus.
• Prior medical office or home care experience preferred.
• Demonstrated record of increased responsibility.
• Ambition to grow and advance beyond current position.
• Strong computer skills; proficiency with electronic medical records.
• Excellent communication and interpersonal skills.

Responsibilities

• Provide superior customer service and quality home care.
• Support the client services manager in coordination of client services and scheduling issues.
• Build lasting relationships with clients, referral sources, payors and community organizations.
• Develop strong, communicative relationships with the team.
• Monitor metrics to support fiscal management (admissions, Medicare %, referrals rejected by reason).
• Partner with Clinical Managers to provide supervision and support to field employees.

Responsibilities for an Associate/ Referral Coordinator:

• Provide superior customer service and quality home care
• Focus on supporting the client services manager in coordination of client services and emergent scheduling issues
• Build lasting relationships with clients, referral sources, payors and community organizations
• Develop strong, communicative relationships with the team
• Support the team in effective fiscal management by monitoring metrics (admissions, % Medicare, referrals rejected by reason, etc.)
• Partner with the team and Clinical Managers to provide supervision and support to field employees

Qualifications for an Associate/ Referral Coordinator:

• Bachelor degree is preferred, Associates at minimum.
