Job summary

Mass General Brigham's Spaulding Rehabilitation Cape Cod is seeking a community outreach professional to develop preadmission connections and education for patients. You will build referral pathways with local providers, facilities, and community partners to support timely access to care.

Responsibilities include educating partners about services, collaborating with physicians and care teams, organizing events, and traveling within the community to expand outreach and improve patient referrals.

Qualifications

• Education in Healthcare Management, Public Relations, or related field is required.
• Experience in community outreach and healthcare/regulatory awareness is preferred.
• Valid driver's license and ability to travel within the community.

Responsibilities

• Build and maintain relationships with key stakeholders to promote services and create referral pathways.
• Educate community partners and healthcare providers about hospital programs and resources.
• Collaborate with physicians, discharge planners, case managers, and other professionals to facilitate referrals.
• Conduct assessments to identify healthcare needs and gaps in the community.
• Organize community events, health fairs, seminars, and workshops to engage the public.
• Collaborate with marketing/public relations to develop targeted outreach materials.
