Job summary

Spaulding Rehabilitation Cape Cod is seeking a dedicated professional to evaluate and manage referrals to develop comprehensive preadmission assessments while meeting productivity goals and ensuring clinical, regulatory, and performance expectations are met.

You will build relationships with community partners, educate healthcare providers about services, and collaborate with physicians, discharge planners, and case managers to facilitate patient referrals and promote outreach across Cape Cod.

Qualifications
