Job summary

SouthEast Alaska Regional Health Consortium (SEARHC) is seeking a care coordination specialist to connect patients with referrals, schedule appointments, arrange travel and housing, and manage insurance authorizations as needed.

This role maintains referrals in electronic records, communicates with patients promptly, and coordinates travel through SEARHC and tribal partners; a high school diploma and customer service experience are preferred.

Qualifications

• High School Diploma or equivalent is required.
• Experience in public interaction by phone or in person is preferred.
• Health care field experience or related education is preferred.

Responsibilities

• Facilitates referrals for patients including insurance authorizations for travel/appointments.
• Inputs data into designated databases and EHR for communication across the consortium.
• Schedules or facilitates appointment scheduling.
• Provides prompt feedback regarding care coordination issues.
• Maintains patient referral information electronically.
• Coordinates travel arrangements for patients through SEARHC and partners.
• Arranges reservations for travel including air, ferry, lodging, and ground transportation.
• Other duties as assigned.

Pay Range

Pay Range:$25.00 - $30.04 Provides care coordination for patients with medical referrals. Care coordination includes the logistical interface that includes making appointments, scheduling, checking alternative resources and facilitating connections to resources, making housing and travel arrangements. Other items directly related to care coordination may be requested through need or delegation.

Key Essential Functions and Accountabilities of the Job

Facilitates referrals for patients including the technical and clerical components of the request(s), including any insurance authorizations that may be required for travel and/or appointments.

Inputs data into the designated database(s) and electronic health record for ease of communication across the consortium.

Schedules or facilitates the scheduling of appointments.

Provides prompt feedback regarding issues related to care coordination, seeking assistance when appropriate.

Maintains patient referral information electronically.

Consults with appropriate staff for questions regarding eligibility status of patients.

Responsible for timely communication with patients regarding care coordination – appointments, scheduling, housing and travel.

Coordinates travel for patients of all ages through SEARHC, the Alaska Tribal Health System, and other external agencies.

Arranges reservations for patient travel, including air, ferry, lodging and ground transportation in compliance with SEARHC policies and procedures.

Other duties as assigned.

Additional Details

Education, Certifications, and Licenses Required High School Diploma or equivalent – required

Experience Required 2 years working with the public in person and on the phone - preferred.

1 year working in the health care field *or* 6 months education in a health care related field - preferred.

Knowledge of Understanding of tribal health programs, referral process and alternate resources

Knowledge and demonstrated use of customer service principles

Knowledge of data entry, retrieval and reporting

Skills in Effective oral and written communication skills

Skills in using a database

Skills in operating a computer utilizing a variety of software applications

Ability to Ability to multi-task and work independently in a fast-paced environment

Ability to respond quickly in urgent situations with attention to detail
