Job summary

AltaMed Health Services in West Covina, CA is seeking a Clinic Referrals Coordinator to manage referral data entry, track and follow up on specialty referrals, and coordinate care for urgent cases. You will work closely with patients, providers, utilization management, and multiple departments to ensure timely access to specialty services.

The role emphasizes accuracy in data entry, proactive communication, and collaboration within a multidisciplinary care team to support high-quality patient

Qualifications

• High school diploma required.
• Minimum of 4 years of experience required.
• Medical Assistant certificate preferred.
• Bilingual English/Spanish/Mandarin/Cantonese preferred.
• A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

Responsibilities

• Enter referral data accurately into the appropriate portal and ensure timely processing.
• Follow up on all specialty referrals and coordinate care for urgent referrals.
• Serve as liaison between patients, providers, utilization management, and other departments to ensure timely access to specialty care.
