AltaMed Health Services in West Covina, CA is seeking a Clinic Referrals Coordinator to manage referral data entry, track and follow up on specialty referrals, and coordinate care for urgent cases. Qualifications

• High school diploma required.
• Minimum of 4 years of experience required.
• Medical Assistant certificate preferred.
• Bilingual English/Spanish/Mandarin/Cantonese preferred.
• A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

Responsibilities

• Enter referral data accurately into the appropriate portal and ensure timely processing.
• Follow up on all specialty referrals and coordinate care for urgent referrals.
• Serve as liaison between patients, providers, utilization management, and other departments to ensure timely access to specialty care.

Job Overview

The Referral Coordinator position is responsible for accurate and timely data entry of referral requests into the appropriate portal. They are also responsible for follow-up on all specialty referrals and coordination of care for urgent referrals. The referral coordinator is an integral part of the care team, working as a liaison between the patient, providers, utilization management, and various departments to ensure timely access to specialty care.

Minimum Requirements

• High school diploma required.

Compensation

$25.00 - $29.32 hourly
