Job summary

Vista Community Clinic (VCC) is seeking a Referral Coordinator to streamline referral workflows across multiple clinics. You will coordinate authorizations, gather necessary medical information, and guide patients through scheduling and follow-up with specialists.

The role requires strong attention to detail, bilingual English/Spanish abilities, and experience with electronic medical records such as NextGen.

Qualifications

• High school graduate or equivalent.
• Minimum two years' experience in a medical, social service setting or related field.
• Bilingual English/Spanish.

Responsibilities

• Coordinate, process, obtain prior authorizations, and request reports for referrals requested by VCC clinicians.
• Organize information from patient records to complete referral authorizations and verify insurance eligibility.
• Provide patients with referral information and how to contact specialists and schedule appointments.
• Act as liaison with referral providers and agencies.
• Document referral information in the EHR using NextGen and HealthViewX (HVX).
• Use referral tracking system to move referrals through Not Started-Pending-Follow-up-Closed and document progress.
• Verify receipt of consults, reports, and ensure they are scanned into patient charts.
