Job summary

Via Care Community Health Center is seeking a Case Manager to help patients understand health care recommendations, perform basic patient care tasks, screen medical calls, and document information in medical records to support treatment planning.

You will coordinate referrals, arrange follow-up appointments, provide health education, and assist with translation and back-office tasks while upholding HIPAA and care standards in a fast-paced clinic.

Responsibilities

• Discharge and arrange referrals or follow-up appointments.
• Perform as a patient advocate and liaison for specialty providers and social service referrals.
• Arrange patients’ appointments for continuity of care and other referrals.
• Provide health education to patients per provider request.
• Adhere to HIPPA guidelines.
• Document follow-up was done on providers’ recommendations.
• Maintain and observe ergonomic safety precautions.
• Attend in-services and/or training.
• Performs clerical duties as assigned.
