Job summary

LifeLong Medical Care is seeking a Referrals Coordinator for the Brookside San Pablo Health Center in San Pablo, CA. This full-time role involves processing specialty referrals, follow-up, data entry, and tracking in Epic and i2i.

The position is 40 hours per week, day shift, with SEIU-UHW representation. A high school diploma or GED and prior administrative experience in medical settings are preferred.

Qualifications

• High school diploma or GED required.
• 1 year of administrative experience in medical or human services setting preferred.
• Experience with electronic health records a plus.
• Proficient in MS Word, Excel, Outlook and internet; typist 40 wpm.

Responsibilities

• Receive referral requests from providers and process them.
• Clarify and communicate with providers; request urgent appointments when needed.
• Verify current insurance information.
• Contact HMO/PPO/Medi-Cal/Medicare or other funding sources for authorizations.
• Follow up on referrals and maintain tracking in Epic and i2i.
• Coordinate with Health Information Technician to place medical reports in charts.
• Performs other duties as assigned by Supervisor.
