Job summary

Susquehanna Health & Wellness Center seeks a Hospital/Clinical Liaison to coordinate the referral process for our skilled nursing facility. You will serve as the primary contact for referrals, working with community hospitals, physicians and Admissions to convert inquiries into admissions.

You should have marketing and sales experience in healthcare, preferably in long-term care, and a Bachelor's degree. The role includes building relationships, meeting targets, and representing the organization

Qualifications

• Bachelor's degree in Business, Healthcare Admin, or Marketing.
• 3–5 years marketing/PR experience in health care.
• Long-term care experience required.
• Advanced training in hospital or nursing facility administration.
• Knowledge of healthcare reimbursement and regulatory standards.
• Excellent oral and written communication.
• Professional sales/presentation skills.
• Ability to work tactfully with staff, residents, families and public.
• Marketing/public relations for nursing facility operations knowledge.
• Background check and drug screen required.

Responsibilities

• Devise strategies to gain patient referrals through relationships with physicians and providers.
• Generate new patient activity and deepen referral source relationships to grow revenue.
• Formulate a sales plan to meet monthly, quarterly, annual targets.
• Establish and monitor sales process to meet goals.
• Execute day-to-day operations to drive KPIs (referrals, conversion, satisfaction).
• One-on-one sales calls and community interactions to raise organizational awareness.
• Act as the key resource for internal and external customers.
• Identify new business opportunities with referring partners.
• Develop census per business plan targets.
• Assist referral-to-admission coordination.
• Represent the organization in community activities.
• Recognize barriers to admission and respond timely.
