Job summary

Concordance’s Post Release Therapist in Charleston, SC joins a multidisciplinary team to support justice-involved participants on their healing journey. You will deliver counseling through CBT and MI, monitor progress, and update treatment plans within a year-long care model.

This full-time role emphasizes compassionate, evidence-based practice, collaboration, and community reintegration. Qualifications include a Master’s in counseling/social work, independent licensure, and 3–5 years in mental

Qualifications

• Master’s degree in counseling, social work, behavioral health, or related field
• Independent clinical license
• 3–5 years of experience in mental health and substance use disorders
• Familiarity with a 12-step program and CBT interventions
• CPR/First Aid certification preferred

Responsibilities

• Facilitating weekly 1:1 and group sessions with CBT and motivational interviewing approaches
• Providing frequent, supportive communications to participants
• Updating participants’ objectives and goals in their treatment plans
• Using assessments to confirm treatment plan goals and progress
• Upholding the Concordance Re-entry Model curriculum in sessions
• Preparing for sessions in advance to maximize effectiveness
• Evaluating participant progress and needed support using assessments
• Documenting sessions in the electronic record system
• Collaborating with care teams and providing input on progress
• Attending internal team meetings and training sessions; assist with center events/setup
