Job summary

The Kings County District Attorney’s Office (KCDA) invites applications for an Outreach Specialist in the Community Resources & Re-Entry Bureau under the KCRTF Grant. The role supports individuals transitioning from incarceration-related environments with case management and direct services.

Under supervision with some initiative, the specialist will conduct intakes, input data, maintain notes, and refer participants to housing, healthcare, and employment services while traveling to NYS

Qualifications

• High school graduation or equivalent and three years of experience in community work or related activities.
• Associate degree and two years of social service work experience.
• Grant-funded position with focus on reentry services

Responsibilities

• Facilitate intakes with prospective REENTRY participants.
• Input intake data into the tracking sheet.
• Maintain intake case notes.
• Case conference with other REENTRY staff to refer participants to appropriate services.
• Participate in ongoing case conferences to ensure continuity and quality of services.
• Provide short-term individualized case management to participants (housing, healthcare, employment, etc.).
• Conduct outreach at 15 2nd Avenue, DOCCS facility.
• Attend community events and resource fairs to share KCDA REENTRY services.
• Travel to NYS Correctional Facilities to facilitate information sessions and reentry planning sessions.
• Track outreach and reentry planning data at NYS correctional facilities.
• Attend monthly planning with Bureau Chief/Executive or Program Administrator to discuss cases.
