Job summary

Coconino County Sheriff's Office (Patrol) is seeking a Reentry Coordinator for Pathways to Community in partnership with Coconino County Health and Human Services. The role coordinates comprehensive case management and reentry navigation for individuals currently incarcerated or recently released, working with on-site partners and service providers to ensure seamless support.

Qualifications

• Associate degree in public/business administration or related field with two years of administrative experience; or
• Three years of administrative experience; or
• Equivalent combination of education, training and experience demonstrating ability to perform duties.

Responsibilities

• Coordinate comprehensive case management and reentry navigation services.
• Collaborate with on-site partners, community providers, and clients’ legal teams.
• Ensure resource connections and continuity of care throughout reentry process.
• Prepare and maintain complex reports and records.
• Manage contracts and communications with partner agencies.
