Job summary

Qualifications

• Requires a four-year degree in social, human services, corrections, or community-based services.
• Minimum one year of experience in human services, corrections, or community-based services.
• Ability to communicate effectively, manage time and multi-task.
• Knowledge of community resources and the local employment market.
• Proficiency with computers and standard software.
• Physically able to reach, bend, and walk during daily routine.

Responsibilities

• Under supervision, manage a caseload and follow program policies and procedures.
• Document program services and ensure records are complete per policies.
• Conduct individual or group sessions for orientation and job readiness.
• Manage daily and weekly accountability plans for clients.
• Advise clients on personal, family, financial, employment, crisis management, health, substance abuse and education matters.
• Maintain client files and complete paperwork in a timely manner.
• Coordinate client requests for permissions to drive, obtain phones, relocation and furloughs.
• Coordinate release planning with the Program Review Team (PRT).
• Prepare release plans, release papers, and related documents.
• Monitor client issues and report to supervisor.
• Coordinate IPP with social service coordinator, client, family and other agencies.
• Recommend remedial action or sanctions for noncompliance.
• Manage client status in EMP and ensure scheduling data is current.
