Job summary

Responsibilities

• Conduct release planning for incarcerated individuals requiring follow-up medical and mental health care.
• Coordinate with medical providers, therapists and community resources for transition planning.
• Complete reentry assessments and health service reentry plans for eligible patients.
• Lead reentry groups and coordinate with DDA/DSHS for community services.
• Document contacts and release planning activities in the agency systems.
• Engage in multidisciplinary meetings to support continuity of care on release.
