Qualifications

• Valid LMSW or LCSW license registered with NYSED.
• Experience in correctional health care and reentry services.
• Ability to coordinate with multiple agencies and teams.

Responsabilités

• Work with interdisciplinary treatment teams to plan discharge for mentally ill patients in correctional settings.
• Collaborate with mental health colleagues for appropriate care levels.
• Assess reentry mental health services and arrange referrals to community providers.
• Assist patients with Medicaid, public assistance, and housing resources.
• Participate in therapy rounds, meetings, and group interventions.
• Complete court-mandated tasks related to reentry duties.
• Coordinate post-release care to community facilities or outpatient services.
