Aufgaben

• Plan and coordinate prerelease services such as home placement and transportation for medical appointments.
• Monitor and facilitate PORTAL New Directions, MR Therapy, and prerelease programming.
• Assist with risk and needs assessment-related program plans for offenders.
• Engage with community providers to support reentry and attend local reentry councils.
• Prepare monthly reports and maintain offender files per ACA standards.
