Job summary

Delaware Alliance for Nonprofit Advancement is seeking a Reentry Navigator (Case Manager) in Georgetown, Delaware. You will manage a caseload of 25–30 high-risk participants returning from incarceration and coordinate housing, health care, and benefits referrals.

Strong writing and collaboration with the Reentry Court team are essential. The preferred candidate has advanced clinical credentials (LCSW/LPC/CADC/LCDP) and a track record in intensive case management, supervision, and CBT-based

Qualifications

• Licensed clinical social work degree or equivalent preferred.
• Minimum three years of intensive social service case management experience.
• Supervisory experience is required.
• Ability to coordinate services via a case plan.
• Strong writing and documentation skills.

Responsibilities

• Provide case management for 25–30 participants returning from incarceration.
• Assess needs using risk assessments; deliver cognitive behavioral strategies.
• Create individualized Achievement Plans aligning with criminogenic risk.
• Conduct weekly 1:1 sessions with participants.
• Coordinate housing, transportation, health care, and benefits referrals.
• Attend Re-entry Court status hearings and document progress.
• Produce clear case notes and data for program evaluation.
