Job summary

The Salvation Army North & Central Illinois Division is seeking a Reentry Case Manager for the Pathway Forward Program in Chicago. You will oversee client intake, assessments, and ongoing planning to support successful community reentry and reduce recidivism.

You will maintain detailed case files, coordinate with partner agencies, and collaborate with program staff to provide comprehensive support services, including employment and educational opportunities.

Qualifications

• Bachelor’s degree in social/human services, criminal justice, behavioral science, or relevant discipline.
• Previous case management and/or counseling experience is preferred.
• Strong communication and organizational skills are essential.

Responsibilities

• Manage intake and assess resident strengths, needs and risk factors for recidivism.
• Maintain and audit case files for accuracy and timeliness.
• Meet residents every 14 days and update supervision plans accordingly.
• Coordinate referrals to job readiness, training programs, and outside agencies.
• Prepare a comprehensive Terminal Report and participate in staffing decisions.
