Job summary

Cherry Health (Cherry Street Services) DBA in Detroit is seeking a dedicated professional for case management and coordination of resident clinical activities in our 24-hour residential reentry program. You will work with the Clinical Supervisor and Manager on clinical issues and ensure treatment plans and aftercare development are implemented.

The role requires collaboration with multidisciplinary teams, participation in program reviews, and adherence to policies, procedures, and licensing

Qualifications

• Bachelor’s degree required in social, human services, criminal justice or behavioral science.
• LMSW or Master’s degree preferred.
• MCBAP certification desired.
• Excellent driving record and current valid driver’s license.

Responsibilities

• Case management on assigned clients including biopsychosocial assessments and treatment plans.
• Individual and group counseling; progress reports to referral sources.
• Coordinate and monitor resident activities; discharge planning and paperwork.
• Consult with Clinical Supervisor/Manager to maintain continuity of treatment.
• Facilitate psychoeducational groups; maintain therapeutic environment.
• Maintain records of resident progress in a timely manner.
• Other duties as assigned.
