Job summary

Cherry Health is seeking a dedicated Case Manager to oversee resident clinical activities at our Detroit Community Treatment Center. You will perform biopsychosocial assessments, develop treatment plans, and lead individual and group counseling sessions while coordinating with supervisors to ensure continuity of care.

The role requires a Bachelor’s degree in a related field, with LMSW/ MSW preferred and MCBAP certification desired.

Qualifications

• Bachelor’s degree in social, human services, criminal justice or behavioral science is required.
• LMSW or Master’s degree is preferred.
• MCBAP certification is desired.
• Excellent driving record and current valid driver’s license.

Responsibilities

• Case management responsibilities on assigned clients, including biopsychosocial assessments, treatment plans, individual and group counseling, progress reports, and discharge planning.
• Consult with Clinical Supervisor/Manager to maintain continuity of treatment and resident activities.
• Coordinate and monitor resident activities within the program treatment model.
• Participate in Program Review Team meetings and clinical case reviews.
• Facilitate psychoeducational groups as assigned.
• Maintain therapeutic environment and provide disciplinary action in consultation with supervisor.
• Maintain records of resident progress in a timely manner.
• Other duties as assigned.
