Job summary

Terros Health in Phoenix, AZ seeks a Behavioral Health Technician for the Bridging the Gap ReEntry Program. The role involves direct care services, intake/assessments, and coordinating connections to community resources for individuals re-entering the community.

The position requires a High School Diploma or GED with behavioral health experience, strong communication skills, and the ability to work independently. Travel up to 80% is possible while working day shifts in multiple locations.

Qualifications

• High school diploma or GED with behavioral health education and/or experience.
• Customer service and excellent oral/written communication are required.
• Ability to work independently with good time management and deadlines.

Responsibilities

• Provide direct face-to-face community-based services to assist re-entry from jail/prison.
• Coordinate referrals, linkages to community resources, and case management as needed.
• Maintain accurate documentation and notes in required databases by midnight daily.
• Transport members as appropriate in personal or company vehicle when available.
• Collaborate with families, providers, and other stakeholders to coordinate care.
