Job summary

Terros Health is seeking a Behavioral Health Technician in Phoenix, AZ, for the Bridging the Gap ReEntry program. The role involves direct care services, intake assessments, and coordinating connections to community resources.

You will report to the BTG ReEntry and Aftercare Supervisor and work across two Phoenix/Mesa locations. The role emphasizes compassionate, professional, and organized engagement with members re-entering the community, with a focus on wellness and recovery-oriented

Qualifications

• Experience providing direct care and in-home support.
• Ability to coordinate referrals and case management.
• Strong record-keeping and documentation skills.

Responsibilities

• Direct face-to-face community-based services to assist clients re-entering the community from jail/prison.
• Coordinate care with family, providers, and community resources.
• Arrange transportation for members as needed.
• Maintain professional records and notes by midnight each day.
• Engage members with recovery-oriented services and wellness strategies.
