The candidate will provide direct services, manage documentation, and collaborate with hospital partners to ensure seamless care. This full-time position runs Tuesday–Saturday, 2:30 p.m.–10:30 p.m., with shift differential potential.

Qualifications

• Bachelor’s degree in social work or related field.
• Experience providing direct services to people experiencing homelessness or similar settings.
• Strong communication and crisis intervention skills.

Responsibilities

• Provide direct services to clients in the Recuperative Care Program.
• Maintain a safe environment and ensure staff coverage in the program.
• Perform comprehensive case management and coordinate with partners.
