Axiom Care is seeking a Peer Support Specialist to work with the treatment team in Phoenix, AZ. The role centers on providing emotional, social, and practical support to individuals in recovery using personal lived experience.

You will lead 1:1 and group sessions, assist with transportation and navigating resources, and maintain HIPAA-compliant documentation and collaboration with other departments.

Qualifications

• Demonstrate strong active listening skills to ensure people feel heard.
• Identify needs and recovery goals for each client.
• Reduce isolation and promote empowerment through peer support.
• Proficient with EMR systems and Microsoft Office.
• Ability to make sound decisions in emergencies.
• Adhere to ethical treatment standards and policies.

Responsibilities

• Build trusting, recovery-focused trauma-informed relationships using lived experience.
• Facilitate psychoeducational groups and support groups.
• Educate clients on harm reduction strategies.
• Transport clients to appointments or services when needed.
• Collaborate with all departments to assess client progress and barriers.
• Attend treatment team staffing, clinical supervision, or other meetings as requested.
• Complete all clinical documentation by end of shift.
