We are looking for a Recovery Support Worker to join our team. The individual in this position will work alongside the clinical team to assist with effective transition and contributes to discharge planning by connecting the client with appropriate aftercare resources within their local recovery community. Responsibilities include:
- Establish and maintain positive, effective relationships with all public agencies within the center’s referral area and continue to develop and foster effective inter-disciplinary communication skills with providers, clients, insurance companies and other interested members of the community.
- Ongoing professional networking and communication with transitional, sober living, and scholarship resources.
- Assist the client in application and interview process with transitional, sober living, and scholarship resources as part of their aftercare plan.
- Assist clients’ applying for financial assistance and navigating Medicaid and other available coverage resources.
- Develop, coordinate and participate in community networking to facilitate client connection to their recovery community and self-help groups.
- Establish and maintain relationships with service agencies and volunteers within the local recovery community to aid in the responsibility of organizing and scheduling guest presentations and in-house recovery / self-help groups.
- Support the client’s clinical team in providing referrals to appropriate community resources, facilitating access and communication when multiple services are involved and coordinating services such as management of benefits, housing, and other basic needs to avoid duplication.
- Work collaboratively with internal clinical and medical providers to ensure high quality care.
- Properly and accurately, communicate observations and other data related to the condition of the client to the primary clinician as appropriate.
- Facilitate didactic and recovery groups as directed.
- Develop, coordinate and ensuring active client participation in on campus and community pro-social activities.
- Maintain documentation for daily logs, progress notes and other required data.
- Attend trainings and follow the employee training plan guidelines.
- Make follow up calls to check on clients post discharge.
Qualifications:
- Minimum High School Diploma or equivalent
- Eligible to obtain NH Certified Recovery Support Worker credentials (CRSW) within 18 months of hire
- Four (4) years’ experience in a related field
- Two (2) years’ customer service experience in a substance use treatment services environment
- Ability to work collaboratively with staff and other service providers, both public and private, on behalf of the clients
- Experience working with culturally and clinically diverse populations.
- Critical thinking and problem-solving skills
- Computer literacy to maintain and manage case records
- Valid driver’s license and reliable transportation
Hours: This is a full-time position.
