Job summary

Qualifications

• High school diploma or GED is required.
• Valid driver’s license with a safe driving record.
• Must have been a recipient of mental health or substance use services and be progressing in recovery.

Responsibilities

• Provide person-centered wellness support to engage individuals in treatment and wellness plans.
• Work with the treatment team, document contacts, and support community integration.
• Offer outreach, daily living support, crisis intervention, and advocacy to families.
• Promote employment, education, health, and meaningful community participation.
