Job summary

InterCommunity, Inc. in Hartford, CT, is seeking a Peer Support Specialist to provide engagement, care coordination, and aftercare planning within the Coventry House Treatment Team.

The role focuses on supporting clients in recovery from mental illness and substance use, connecting them with clinical, medical, social, educational, rehabilitative, and vocational services. Requirements include Recovery Support Specialist training or equivalent experience and CPR certification within 60 days.

Qualifications

  • Recovery Support Specialist training or equivalent experience in a recovery setting.
  • Experience serving individuals in recovery from mental illness and substance use is preferred.
  • CPR certification within 60 days of start date.

Responsibilities

  • Engage clients and support ongoing attendance by completing tracking documents and reports.
  • Collaborate with clinical teams to identify needed services and coordinate care.
  • Provide linkage to community resources and support treatment completion with aftercare planning.
  • Assist in crisis assessment and facilitate admission to detox when needed.
  • Document treatment activities in accordance with procedures.

Summary:

The Peer Support Specialist holds a certificate from a certified Recovery Support program and provides skill building, engagement, and care coordination as part of the Coventry House Treatment Team. The main focus of the PeerSupport Specialist supports client engagement in treatment and treatment completion with an aftercare plan.

  • Completed continuing education credits to maintain Recovery Support Specialist Certificate annually.
  • Focus on client engagement and encouragement in treatment with a goal of supporting treatment completion.
  • Assist in providing each client access to necessary clinical, medical, social, educational, rehabilitative, vocational and/or other services. Provide linkages with various community resources. ?
  • Collaborate with the clinical team and their clients to identify required services by assessing the overall needs of each client.
  • Support client recovery by joining OP clinicians, Primary Care and participating in individual treatment sessions as necessary.
  • Completes all required documentation related to treatment in accordance with established procedures including but limited to court letters, gathering medical records, copying, faxing, scanning, collateral contact and appointment scheduling.
  • Engage clients and support ongoing attendance by completing all tracking documents and reports to support engagement
  • Provide assistance during the assessment of individuals in crisis and intoxicated clients by helping to determine the need for crisis involvement or detox and facilitate the admission to detox facility.
  • Provides education, support and consultation to individual’s families and their support system which is directed exclusively to the well-being and benefit of the individual, if needed.
  • Performs related duties as required including transportation of clients. Must have a reliable and insured vehicle.

*All agency staff are required to attend all mandatory department/agency meetings and trainings*

Schedule:

Tuesday - Saturday, 10:00 AM - 6:30 PM

Requirements

Recovery Support Specialist training through Advocacy Unlimited. Equivalent experience in a community health center or behavioral health setting may be considered. Experience working in a setting attending to the needs of those in recovery from mental illness and substance use/addiction.

All Residential Support Staff Employees are required to show proof of a CPR Certification within 60 days of the official start date. ??
