If you are a CRS/CFRS, or meet the requirements to pursue certification with the Pennsylvania Certification Board, this could be a great opportunity for you to to help others in a unique professional role.ESSENTIAL DUTIES AND RESPONSIBILITIESPerforms work associated with recovery support services, case management, treatment and rehabilitation of persons with drug and/or alcohol use. Services and support will be delivered to patients in the primary care setting in conjunction with Medication Assisted Treatment (MAT).In accordance with Patient Centered Medical Home philosophy, collaborates with team members including: patient; primary care provider; behavioral health provider and others to deliver comprehensive care to patients of Family First Health through:Patient Engagement and EmpowermentParticipates in care coordination by utilizing case management tools, such as strength-based case management, motivational interviewing to facilitate patient retention in care and move patients toward independence and increased autonomy.Assists participants in the management of their recovery by ensuring that the resources to address the participant’s needs are in place.Provides drug and alcohol education, risk reduction support, and medication adherence guidance.Promotes participant retention, self-sufficiency in treatment and utilization of available support through educating participants about service availability.Engages in brief relationship building and active listening with patients receiving treatmentProvides supportive (peer-based) coaching and encouragement to patients contemplating or actively seeking help with substance use treatment and recovery.Supports patients in developing a self-directed recovery plan to meet their recovery goalsWill serve on-call duties based on on-call policy and departmental procedures.Connection with other providersIdentifies, develops and maintains positive linkages with agencies and social service organizations/services in the community and within FFH.Communicates with FFH interdisciplinary team, peers in other departments, managers, patients and other community agencies to ensure ongoing care for patients, maintain connectedness, and foster positive internal/external customer service.Facilitates warm handoffs between all available behavioral health and physical/dental health treatments.Creates and manages long-term individualized service plan by assessing patient’s target needs and reassess on timely basis, including making appropriate referrals for treatment and rehabilitation of drug and alcohol dependent patients.DocumentationMaintains timely and accurate documentation and tracking of all service provision, evaluation, and statistical data for the program; including detailed patient demographics, medical information, risk factors, and other information needed related to program funding.Understands and adheres to all state and federal drug & alcohol confidentiality laws and related statutes.Attends and actively participates in both internal and external meetings as required.Attends trainings and conferences as determined by supervisor.Performs other duties as assigned and related to the position.Must have a minimum of two (2) years uninterrupted recoveryMust have or be eligible to pursue Certified Recovery Specialist (CRS) or Certified Family Recovery Specialist (CFRS) credentialMust be actively engaged in activities that support a recovery oriented lifestyleHigh School Diploma or GED required.
