Job summary

Horizon Behavioral Health in Lynchburg, VA seeks a Case Manager - Reentry Grant to manage a caseload of justice-involved adults with substance use disorders. You will develop reentry plans, coordinate evidence-based treatment, and ensure continuity of care from incarceration to community reintegration.

The role requires a Bachelor’s degree and at least 1 year in mental health/substance use work, plus a valid Virginia driver’s license for travel to Horizon sites.

Qualifications

• Bachelor’s degree from an accredited college or university in an applicable human services field or equivalent education.
• Minimum of one year of relevant mental health, developmental disability, and/or substance use work experience with the identified population.
• Valid Virginia driver’s license and ability to travel to Horizon sites.

Responsibilities

• Manage an assigned caseload of justice-involved adults with SUD and co-occurring conditions.
• Develop individualized reentry and service plans and coordinate evidence-based treatment.
• Ensure continuity of care from incarceration through community reintegration.
• Collaborate with correction staff, behavioral health providers, peers, parole, housing and community partners.
