Job summary

Community Health Associates is seeking a Peer Support Specialist to work with adults and children, providing unconditional, nonjudgmental support and leveraging lived recovery experience.

You will deliver both individual and group sessions on-site and in members' homes, help identify goals, navigate resources, and coordinate with the treatment team to ensure consistent care. Bilingual abilities encouraged.

Qualifications

• Must have firsthand knowledge of behavioral health or recovery.
• Peer Support training completed within 6 months.
• Valid driver's license and reliable transportation.
• CPR/First Aid certification within 30 days of employment.
• Excellent patient-centered communication.

Responsibilities

• Provide peer support to engage members in recovery partnerships.
• Offer individual and group sessions on site and in homes to meet goals.
• Identify, navigate, and use community resources.
• Transport members to appointments as appropriate.
• Assist in setting personal recovery goals and tracking progress.
• Collaborate with the treatment team to plan steps toward goals.
• Document services in behavioral health records per agency rules.
• Advocate for members to improve access to services.
