Job summary

Keystone is a non-profit organization serving Rock Hill, South Carolina, seeking Peer Support Specialists to work varied shifts from Monday–Thursday and Friday, and another from Tuesday–Saturday to support recovery services and treatment coordination. Qualifications

• Bachelor’s degree from a regionally accredited institution preferred; a minimum of a high school diploma or GED is required.
• One year of active participation in a local or national self-help group of recovery movement.
• Completion of a pre-certification program: Forty hours of SCDHHS approved training and achievement of a passing score on the written certification exam.
• Experience and training in working with specific client groups.
• Ability to effectively communicate in oral and written form.
• Proficiency with MS office computer software applications.

Responsibilities

• Demonstrates recovery expertise – including knowledge of approaches to support others in recovery and dual recovery – as well as the ability to demonstrate his or her own efforts at self-directed recovery.
• Conducts care management for patients in collaboration with the clinical team throughout the treatment process.
• Assesses patient needs and provides access to housing, education, employment, childcare, transportation, legal services and any other needs, including Medication Assisted Treatment.
• Maintains effective communication with community partners through staffing and case review.
• Maintains patient record in compliance with all governing bodies to include DAODAS, DHEC, CARF, DHHS (Medicaid) and DSS.
• Remains current by attending Safety, receive an annual TB screening, and mandatory 20 hours training for re-credentialing annually.
• Other duties as assigned by Supervisor.
