Job summary

Community Partners NH is seeking a Peer Specialist to join our ACT Team in providing recovery-focused support to adults with severe mental illness. You will foster supportive relationships, help develop Wellness Action Plans, and connect clients with community resources.

Role requires lived experience or prior peer work, excellent communication skills, and the ability to use technology. Evening/weekend availability may be required to assist clients in crises, with a strong commitment to

Qualifications

• Lived experience of mental illness and recovery and/or has worked in a Peer Support Center.
• Intentional Peer Support Training and WRAP Training is preferred but not required.
• Excellent oral and written communication skills.
• Ability to utilize technology provided to perform requirements of the job.
• Hold a valid driver’s license and proof of vehicle liability coverage.

Responsibilities

• Support adults with severe mental illness as part of the ACT Team.
• Develop Wellness Action Plans with clients and connect them to community resources.
• Advocate for peers and facilitate open dialogue.
• Ensure timely documentation and required reporting.
