Job summary

Change Grow Live in Peterborough is seeking an Outreach Worker to engage with service users facing rough sleeping and substance misuse, guiding them to the right interventions.

You will assess, screen, plan recovery, and refer onward, supporting individuals through entry to services and recovery journeys while promoting empowerment and collaboration with partners. This is a permanent full-time role, 37.5 hours per week.

Qualifications

• Strong understanding of substance misuse issues and experience in a similar field.
• Excellent verbal and written communication with good IT skills.
• Proactive, able to use own initiative with strong time management and prioritising skills.
• Knowledge of mental health interventions and services.

Responsibilities

• Supporting service users from entry into the service and through treatment/recovery journey.
• Providing screening, assessment, recovery planning and onward referral.
• Reducing drug and alcohol-related harm to service users and the wider community.
• Promoting involvement of carers, service users and the community.
• Advocating for access to partnership services.
• Helping service users with social reintegration and empowerment.
