Santa Fe Recovery Center, Inc. is seeking a qualified Peer Support Worker to join our interdisciplinary team. The role focuses on coordinating clientsare from admission through discharge, ensuring effective use of community resources and compliance with external agencies and referral sources.

The Case Manager facilitates success in treatment and prepares clients for reentry into their community, while upholding confidentiality and delivering person-centered care.

Qualifications

• Experience in addiction treatment settings preferred.
• Valid NM drivers license and clean driving record required.
• Case Management certification required (ACM/ CCM/ CPSW).
• Ability to maintain confidentiality per HIPAA.

Responsibilities

• Assess clients on admission to determine goals and support needed.
• Follow up with clients to ensure basic needs are addressed.
• Coordinate resources and referrals to support discharge planning.
• Liaise with family and community services to maintain continuity of care.
• Educate participants on behavioral health and resources.
• Prepare aftercare plans and maintain accurate documentation.
