Job summary

SOUTHWEST BEHAVIORAL CARE, INC. in Greensburg, PA is seeking a client services liaison who assesses service needs and links individuals and families to health and social services, focusing on overcoming barriers to substance use treatment and recovery.

You will coordinate referrals, monitor participation, maintain case records, and support clients through training, on-call coverage, and adherence to laws and confidentiality.

Qualifications

• Must have an associate degree or equivalent in a related field.
• Ability to learn and use an electronic health records system.
• Knowledge of substance use and mental health programs available locally.
• Ability to obtain vital interview information to make service recommendations.
• Must be eligible for necessary federal clearances.

Responsibilities

• Acts as the single point of contact for linking and referral to health and social services.
• Links individuals to recovery-focused supports and services.
• Identifies barriers to treatment and guides navigation through barriers.
• Monitors participation and provides support to encourage engagement.
• Assists with addressing social determinants of health affecting treatment access.
• Coordinates contacts with referral sources and represents the program at trainings and meetings.
• Maintains client records and submits required reports accurately and timely.
• May participate in staff on-call rotation to assist clients remotely after hours.
• Adheres to policies, laws, and confidentiality requirements.
