Job summary

Austin Travis County Sobriety Center is seeking a Care Coordinator (BSW, LCSW, LMSW, LCDC preferred) to support clients on their journey toward harm reduction and recovery. You will collaborate with a multidisciplinary team to assess needs, provide compassionate care, and connect clients with the next steps of their journey.

Qualifications

• Bachelor or Master in Social Work preferred.
• ER or healthcare setting experience preferred.
• Must independently obtain and maintain a valid American Heart Association Basic Life Support (BLS) certification.
• Hold a Texas Driver’s License is required.

Responsibilities

• Complete discharges of clients leaving the center.
• Conduct substance use assessment on clients discharging from the Sobering Center.
• Develop care and recovery plan for persons who request assistance.
• Provide education and brief intervention using Motivational Interviewing to facilitate engagement in SUD services.
• Coordinate with area service providers to help clients access substance use treatment services.
• Connect clients to appropriate community resources.
• Assist clients in overcoming barriers to treatment (transportation, lack of insurance); enroll into MAP when required.
• Conduct risk assessments to identify and manage safety concerns (relapse, self-harm, suicidal ideation).
• Implement crisis intervention strategies and safety planning.
• Collect and enter client information into database.
• Maintain accurate, timely documentation of assessments, care plans, progress notes, and referrals.
• Assist clients with ADLs to ensure dignity and safety.
• Perform related work as assigned.
