Job summary

Wedgwood Christian Services in Michigan seeks a Case Manager to provide intensive case management in the community, coordinating resources across agencies and delivering education on Medicated-Assisted Treatment and related topics.

Responsibilities include developing plans, engaging families, tracking data for funding, ensuring timely paperwork, coordinating aftercare, and supporting QA processes while mentoring new staff.

Qualifications

• Bachelor’s degree in Social Work, Psychology or related human service field.
• Experience working with adolescents and/or families preferred.
• Experience on an interdisciplinary team in a residential treatment setting preferred.
• Must subscribe to Wedgwood Christian Services' Statement of Faith.

Responsibilities

• Develop case management plans in coordination with Clinical Supervisor and team.
• Coordinate resources across agencies and conduct client meetings.
• Provide education to clients and families on MAT and related topics.
• Develop quarterly case plans and facilitate quarterly Case Review Meetings.
• Track information from intake through aftercare for program data and funding.
• Ensure timely authorizations and documentation per county/state contracts.
• Coordinate aftercare at discharge to maintain continuity of care.
• Participate in quality assurance and staff development activities.
• Support training and mentoring for new hires.
