Qualifications

• High School or GED Equivalent as essential education.
• 1 year directly related experience.

Responsibilities

• PATIENT SAFETY: Follow safety policies, procedures and protocols.
• PATIENT SAFETY: Improve patient safety by questioning current policies and processes.
• PATIENT SAFETY: Identify and report environmental conditions that pose risk.
• PATIENT SAFETY: Report potential safety concerns and medical errors promptly.
• PATIENT SAFETY: Encourage patient participation in care and questions.
• PATIENT CENTERED MED: Promote core expectations of patient-centered care.
• NAVIGATION/PATIENT ADVOCACY: Help patients access resources and services.
• COLLABORATION: Work with providers and community resources to meet goals.
• LIFE SKILLS: Develop skills in daily living and community functioning.
• DEVELOPMENT: Plan for professional growth and learning.
• WELLNESS: Provide information on illnesses and medications; promote recovery.
• WELLNESS: Discuss medications and side effects with providers.
• SERVICE PLANS: Develop and coordinate service plans and crisis plans.
• RESOURCE COORD: Assist access to services, housing, and benefits.
• SUPPORT SERVICES: Encourage development of natural supports.
• ASSESSMENT: Identify strengths, needs, and barriers; ongoing review.
• CRISIS INTERVENTION: Assess and intervene in crises; plan relapse prevention.
• TRACKING: Monitor service use and follow up post-discharge.
• DOCUMENTATION: Maintain and submit client documentation; file management.
• STATISTICS: Maintain and report program statistics.
• DEVELOPMENT: Engage in ongoing professional development.
