Job summary

UNM Hospital seeks a healthcare professional to coordinate services and resources for individuals and families to promote rehabilitation, recovery, and resiliency. The role focuses on identifying strengths and barriers to support independent community functioning, including crisis situations and personalized interventions.

The position spans age groups from neonate to geriatric and requires collaboration with medical providers and community resources to meet treatment goals and ensure adherence

Qualifications

• Education and Experience may be substituted year-for-year.
• High School or GED Equivalent.
• 1 year directly related experience.

Responsibilities

• PATIENT SAFETY - Follow patient safety-related policies, procedures and protocols.
• PATIENT SAFETY - Demonstrate proactive approach to patient safety by seeking opportunities to improve patient safety through questioning of current policies and processes.
• IDENTIFY BARRIERS - Identify and report environmental conditions and/or situations that may put a patient at undue risk.
• PATIENT SAFETY - Report potential or actual patient safety concerns, medical errors and/or near misses in a timely manner.
• PATIENT CENTERED MED - Adhere to and promote the core expectations of the Patient Centered Medical Home or Patient Centered Specialty Practice as applicable.
• NAVIGATION/PATIENT ADVOCACY - Work to eliminate barriers to access to culturally competent medical services for all patients.
