Job summary

The Doe Fund Inc. in Bronx, NY is recruiting a Case Manager to provide supportive case management for residents in a new permanent supportive housing program.

You will work with individuals referred by HASA, helping them access services and achieve independence. The role requires a Bachelor’s in Social Work or related field and at least three years post-Master's experience, with strong knowledge of HIV/AIDS, substance use, mental health, and homelessness.

Qualifications

• Bachelor’s Degree in Social Work or relevant field.
• Minimum of three years of post-Master's experience.
• Knowledge of HIV/AIDS, substance abuse, mental illness, homelessness.
• Strong case management skills and knowledge of benefits in NYC Metro area.
• Sensitivity to community issues and needs.
• Outstanding interpersonal and written communication skills.
• High ethical standards, confidentiality and integrity.
• Proficient in email, Word, Excel and PowerPoint.

Responsibilities

• Maintain a caseload and provide supportive case management.
• Keep resident records compliant with standards.
• Conduct and review assessments.
• Develop and implement individual service plans.
• Assist residents with accessing healthcare providers.
• Provide counseling and crisis intervention when needed.
• Help residents secure and maintain benefits.
• Coordinate with hospitals and community programs.
• Facilitate groups promoting health and independent living.
• Monitor rent payments and budgeting for residents.
• Educate residents about mental health approaches.
• Attend trainings and complete required reports.
• Other duties as assigned.
