At Wyandot Behavioral Health Network (Wyandot BHN), we believe everyone deserves the opportunity to live a healthier, more fulfilling life. Our team is passionate about helping individuals overcome challenges, build on their strengths, and achieve greater independence.

We're looking for a compassionate, motivated Case Manager to join our community-based team serving adults living with severe and persistent mental illness. This is an opportunity to do meaningful work every day by building trusting relationships, connecting people with essential resources, and helping individuals navigate life's challenges.

If you're passionate about helping others, enjoy building relationships, and want a career where your work has a lasting impact, we'd love to have you join our team.

What You'll Do

As a Case Manager, you'll be a trusted advocate and partner in each individual's recovery journey. You'll help clients identify their goals, overcome barriers, and access the services and support they need to thrive.

  • Build meaningful relationships with individuals and families through compassionate, respectful, recovery-focused support.
  • Empower individuals to reach their goals by conducting strengths-based assessments and developing personalized service plans.
  • Coordinate care and services with behavioral health providers, medical professionals, hospitals, families, and community partners.
  • Connect clients with resources for housing, healthcare, employment, transportation, financial assistance, and other essential needs.
  • Advocate for individuals when working with healthcare providers, courts, government agencies, and community organizations.
  • Support clients through challenges and crises by coordinating appropriate services and helping them navigate difficult situations.
  • Monitor progress and celebrate growth by evaluating individual needs and adjusting service plans to support continued recovery.
  • Maintain accurate documentation that meets agency, Medicaid, and regulatory requirements.
  • Collaborate and continue learning through team meetings, clinical supervision, training, and professional development.
  • Work in the community by meeting clients where they are and, when appropriate, providing transportation to appointments and services.
What Makes You Successful

You’ll thrive in this role if you’re someone who values relationships, approaches challenges with patience and empathy, and believes in helping others recognize their potential.

Success in this position includes:

  • Providing person-centered services that promote recovery, stability, and independence.
  • Helping clients navigate complex systems and access the right resources.
  • Building positive relationships with clients, families, and community partners.
  • Balancing client needs with timely documentation and productivity expectations.
  • Working independently while contributing to a supportive, collaborative team.
  • Demonstrating professionalism, cultural awareness, sound judgment, and respect for every individual.
Requirements

What We're Looking For

  • Bachelor's degree in Social Work, Psychology, Rehabilitation, Human Services, or a related field (or equivalent education and experience in the human services field).
  • Valid driver's license, reliable transportation, and current automobile insurance.
  • Strong interpersonal, communication, organizational, and time management skills.
  • Ability to work independently while collaborating effectively within a multidisciplinary team.
  • Basic computer proficiency and the ability to use electronic health records and other required technology.
Preferred Qualifications
  • Experience providing case management or care coordination in behavioral health or human services.
  • Knowledge of severe and persistent mental illness, recovery-oriented services, trauma-informed care, and evidence-based practices.
  • Familiarity with Medicaid requirements and community behavioral health resources.
  • Understanding of regulations and best practices related to community mental health services.
