Community Bridges, Inc. is seeking a Specialty Case Manager to deliver community-based case management for individuals with serious mental illness or high-risk acuity.

You will plan, connect, advocate, coordinate, and monitor services to ensure access to health care, housing, employment, education, and social supports. The role requires a HS diploma or GED (associates preferred), 1 year of behavioral health outreach experience, valid AZ driver license, and related clearances.

Qualifications

• Highschool diploma or GED is required; an associate degree or higher in a behavioral health field is preferred.
• 1 year of experience in a behavioral health position with outreach and engagement is required.
• AZ driver license and related clearances are required or preferred per policy.

Responsabilités

• Provide community-based case management for patients with serious mental illness or high risk due to acuity.
• Assist in planning, connecting, advocacy, coordination and monitoring to access health and social services.
• Document services accurately and timely while promoting a recovery-focused environment.
