Job summary

Bridges of Colorado is seeking a Bridges Service Professional to provide person-centered, solution-focused case management for participants with serious mental health needs. You will conduct intakes and assessments, connect participants to behavioral health and social supports, and track outcomes in the Bridges system.

Qualifications

• Bachelor’s degree in social justice, social work, sociology, criminology, or related field.
• Minimum 3+ years’ experience in criminal justice, behavioral health, or related field.
• Demonstrated alignment with Bridges values, equity, and inclusion.
• Lived experience substitutions may be considered for qualifications.

Responsibilities

• Conduct intake, screenings, and assessments with participants in various settings.
• Provide connection to services (case management, behavioral health, housing, employment).
• Maintain accurate case notes in Bridges case management system and report data as requested.
• Engage in onboarding, team meetings, and professional development as required.
