Job summary

Holcomb Behavioral Health Systems is seeking a Case Manager to provide community-based services to individuals eligible for behavioral health services, including assessing and monitoring resources to help clients live safely in the community.

Schedule: Monday–Friday, 9:00 am–5:00 pm in Kent County, with a 21.00 USD/hour pay rate. The role carries a caseload of up to 30 clients and focuses on recovery-oriented, strength-based service planning and linkage to housing, healthcare, and employment.

Qualifications

• Bachelor’s degree in a relevant field or RN license or equivalent experience.
• Experience in case management or mental health services preferred.
• Ability to develop strength-based, measurable service plans.

Responsibilities

• Carry a caseload of up to 30 clients.
• Perform case management duties for consumers in Behavioral Health Services.
• Provide services based on Recovery Model principles.
• Develop individualized, measurable service plans.
• Assist consumers in obtaining housing, healthcare, employment, and social supports.
• Maintain professional relationships with families, payers, and providers.
• On-call rotation 24/7.
