Job summary

St. Luke's is seeking a Certified Peer Specialist to join its Assertive Community Treatment (ACT) Team. The role focuses on outreach, monitoring, assessment, service planning and family consultation to support recovery and community integration.

Responsibilities include in-home and community visits, documentation in the EHR, crisis on-call duties, and cross-training with ACT specialties to expand housing and vocational supports. High school diploma and CPS certification are required.

Qualifications

• Must hold and maintain Certified Peer Specialist certification through the Pennsylvania Certification Board.
• Dual CPS/CRS certification welcome.
• WRAP (Wellness Recovery Action Plan) certification preferred.
• High School Diploma or equivalent required.

Responsibilities

• Meet with ACT participants in homes and community venues to provide in vivo services.
• Provide medication service including delivery of medications and observation of ingestion as directed by the team.
• Collaborate with ACT staff to develop, write, implement, evaluate, and revise treatment goals.
• Provide on-call crisis services on a rotating schedule as directed by the Team Leader.
• Complete documentation of all services in the electronic health record in a timely manner.
• Work weekends, evenings and holidays on a rotating schedule.
• Cross-train with other ACT Specialties to provide housing, vocational and other supports.
• Educate ACT participants, staff, families and community members about recovery and hope.
• Coach participants in identifying strengths, challenges and goals.
• Provide assessments of past and current experiences with behavioral health services and assist in service plan development.
• Lead educational and support groups using WRAP when properly trained.
